ABSTRACT

Objective:To establish an HPLC-DAD-ELSD method for the simultaneous determination of eight main active components in Buyang Huanwu Decoction, including hydroxysafflor yellow A, paeoniflorin, calycosin glycoside, ferulic acid, ononin, calycosin, fermononetin and astragaloside.Methods:Agilent Eclipse XDB-C18 column (250 mm×4.6 mm, 5 μm) was used with acetonitrile-0.1% formic acid as the mobile phase. The flow rate was 1.0 ml/min; the column temperature was 30 ℃; the detection wavelengths were 230 nm (paeoniflorin), 254 nm (calycosin glycoside, ononin, calycosin, fermononetin), 322 nm (ferulic acid) and 403 nm (hydroxysafflor yellow A); the drift tube temperature of the evaporative light scattering detector was 60 ℃; the carrier gas flow rate was 1.6 L/min.Results:Under these conditions, the separation of hydroxysafflor yellow A, paeoniflorin, calycosin glycoside, ferulic acid, ononin, calycosin, fermononetin and astragaloside was good, and the linear relationship was in line with the requirements ( r=0.994 0-0.999 9). The average recovery was 97.8% - 101.4% ( RSD was 1.28% - 3.70%). Conclusion:The method is simple, stable and reproducible, and can be used for the quality control of Buyang Huanwu Decoction.

ABSTRACT

Objective:To evaluate the effect of Banxia Baizhu Tianma Decoction combined with modified Buyang Huanwu Decoction and routine western medicine on ischemic stroke.Methods:Prospective cohort study. According to random number table method, 120 patients with ischemic stroke who met inclusion criteria in the hospital were divided into control group and treatment group, 60 in each group. The control group was given routine western medicine, while the treatment group was additionally given Banxia Baizhu Tianma Decoction and modified Buyang Huanwu Decoction. All were treated for 4 weeks. The severity of neurological deficits was evaluated by National Institute of Health Stroke Scale (NIHSS). The activities of daily living were evaluated by Barthel index. The high blood shear viscosity (HWBV), low blood shear viscosity (LWBV), plasma fibrinogen (FIB) and plasma viscosity (PV) were detected by full-automatic blood rheometer. The levels of MDA, SOD and NO were detected by ELISA. The clinical responsive rate was assessed.Results:The differences in total response rate between treatment group and control group were statistically significant [93.3% (56/60) vs. 75.0% (45/60), χ2=7.56, P=0.006]. After treatment, NIHSS score in treatment group was significantly lower than that in control group ( t=2.38, P=0.019), while Barthel index was significantly higher than that in control group ( t=13.28, P<0.01). After treatment, HWBV [(5.02±0.13) mPa?s vs. (6.18±0.28) mPa?s, t=29.11], LWBV [(1.18±0.21) mPa?s vs. (1.73±0.32) mPa?s, t=11.13], FIB [(2.26±0.28) g/L vs. (3.13±0.39) g/L, t=14.04] and PV [(8.87±1.44) mPa?s vs. (10.34±1.31) mPa?s, t=5.85] in treatment group were significantly lower than those in control group ( P<0.01), and the MDA [(4.14±1.23) mmol/L vs. (5.23±1.35) mmol/L, t=204.30] in treatment group was significantly lower than that in control group ( P<0.01), and levels of SOD [(113.34±0.28) mg/L vs. (96.59±0.57) mg/L, t=4.62] and NO [(26.01±3.26) μmol/L vs. (20.84±3.74) μmol/L, t=8.07] in treatment group were significantly higher than those in control group ( P<0.01). Conclusion:The Banxia Baizhu Tianma Decoction combined with modified Buyang Huanwu Decoction and routine western medicine can repair nerve function, improve hemorheology, oxidative stress indexes, clinical curative effect and activities of daily living in patients with ischemic stroke.

ABSTRACT

Objective:To explore the curative effect of Buyang Huanwu Decoction combined with suspension exercise training for the patients with cerebral infarction combined and lower limb hemiplegia.Methods:According to random number table method, 94 patients with cerebral infarction and lower limb hemiplegia meeting the inclusion criteria were divided into the control group and the observation group between January 2017 and February 2021, 47 in each group. The control group was treated with suspension exercise training, while observation group was additionally treated with Buyang Huanwu Decoction on the basis of the control group treatment. All were treated for 6 weeks and then followed up for 3 months. Before and after treatment, the Traditional Chinese Medicine (TCM) syndromes scores were recorded. The severity of nerve function injury was evaluated by National Institutes of Health Stroke Scale (NIHSS). The lower limb function was evaluated by Fugl-Meyer Assessment (FMA). The balance function was evaluated by Berg Balance Scale (BBS). The activities of daily life were assessed by Barthel Index (BI). The whole blood high shear viscosity, plasma viscosity and hematocrit were detected by full-automatic hemorheology analyzer. The levels of plasma total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) were detected by full-automatic analyzer. All were followed up for 3 months. The prognosis of patients was assessed by modified Rankin scale (mRS). The adverse reactions during treatment were recorded.Results:After treatment, scores of TCM syndromes and NIHSS in observation group were significantly lower than those in control group ( t=5.35, 4.54, P<0.01), while scores of FMA, BBS and BI were significantly higher than those in control group ( t=3.40, 3.10, 7.57, P<0.01). The whole blood high-shear viscosity, plasma viscosity and hematocrit in observation group were significantly lower than those in control group ( t=2.94, 3.81, 4.23, P<0.05 or P<0.01), and levels of TC, TG and LDL-C were significantly lower than those in control group ( t=4.10, 4.27, 3.61, P<0.01). The differences in good prognosis rate between observation group and control group were statistically significant [74.47% (35/47) vs. 51.06% (24/47); χ2=5.51, P=0.019]. Conclusion:The Buyang Huanwu Decoction combined with suspension exercise training can relieve clinical symptoms, recover lower limb function, improve activities of daily life and prognosis in cerebral infarction combined with lower limb hemiplegia.

ABSTRACT

Objective:To observe the effect of Buyang Huanwu Decoction on renal oxidative stress and NF-κB pathway in diabetic nephropathy rats.Methods:Sixty SD rats were randomly divided into normal group, model group, low, medium, and high dose of Buyang Huanwu Decoction groups and metformin group, with 10 rats in each group. The normal group was given ordinary diet every day, and the other groups were given high-fat and high-sugar diet for 6 weeks, then intraperitoneally injected 35 mg/kg STZ to establish diabetic nephropathy model. After successful modeling, the rats were orally injected with 7.42, 14.84 and 29.68 g/kg of low-dose, medium-dose and high-dose Buyang Huanwu Decoction and metformin group was orally given metformin solution 0.2 g/kg. After 6 weeks of feeding, the body weight, 24h urinary protein, serum creatinine and urea nitrogen were detected. The levels of GSH-Px, SOD, CAT and MDA in rats' kidney of each group were detected. The protein expression levels of NF-κBp65, p-NF-κBp65, TNF-α and IL-6 in rats' kidney in each group were detected by Western Blot.Results:Compared with model group, the body weight of the low, medium and high dose of Buyang Huanwu Decoction groups were increased ( P<0.01), the content of 24 h urinary protein, serum creatinine and urea nitrogen were decreased ( P<0.05 or P<0.01),the content of GSH-Px , SOD, CAT were significantly increased ( P<0.05 or P<0.01), the content of MDA was significantly decreased ( P<0.05 or P<0.01), the expression of NF-κB p65 (0.53±0.07, 0.46±0.09, 0.42±0.10 vs. 0.67±0.13), p-NF-κB p65 (0.51±0.12, 0.43±0.06, 0.34±0.17 vs. 0.59±0.07), TNF-α (1.21±0.08, 1.17±0.04, 1.05±0.22 vs. 1.43±0.21), IL-6 (0.92±0.04, 0.89±0.25, 0.73±0.09 vs. 1.06±0.08) were significantly decreased ( P<0.05 or P<0.01). Conclusion:Buyang Huanwu Decoction can improve oxidative stress state and inflammatory damage in renal tissue. The mechanism may be related to the inhibition of NF-κB signaling pathway.

ABSTRACT

Objective:This study aimed to evaluate the neuroprotective effect of Buyang-Huanwu Decoction (BYHWD) against cerebral ischemia by using magnetic T2WI and DTI to observe the infarct volume, formation of cerebral edema and injury of white matter fibers on cerebral ischemic rats. Methods:Rat model of cerebral ischemia was established by middle cerebral artery occlusion (MCAO). The successfullly modeled rats were randomly divided into model group and BYHWD group, with 5 rats in each group, and the other 5 rats were taken as sham operation group. The Rats were intragastrically administrated with BYHWD (16.1 g/kg) once daily for 30 d after MCAO. T2WI and DTI examinations were performed on the 3rd, 7th, 14th, 30th day after the surgery.Results:T2WI showed abnormal hyperintensities in the right hemisphere of the MCAO rats on the 3rd, 7th, 14th, 30th day after the surgery. The infarction percentage reduced with time coursing ( P<0.05). Compared with the model group, the rats treated with BYHWD showed reduced infarction percentage [3 d: (13.9% ± 13.7% vs. 40.1% ± 10.7%); 7 d: (13.9% ± 11.9% vs. 28.2% ± 7.7%); 14 d: (10.2% ± 7.9% vs. 24.5% ± 3.5%); 30d: (6.8% ± 6.5% vs. 24.7% ± 8.7%)] ( P<0.05 or P<0.01). The percentage of edema reached the peak on the 3rd day after MCAO. Due to the cerebral atrophy on 30th day, the percentage of edema exhibited negative growth. The edema percentage of BYHWD group reduced significantly on the 3rd and 7th day compared with model group [3 d: (11.4% ± 6.9% vs. 21.5% ± 3.1%); 7 d: (5.5% ± 3.1% vs. 8.7% ± 1.2%)] ( P<0.05 or P<0.01). DTI showed that the low signals indicating fiber injuries were observed in the infarct areas of the model group rats on the 3rd, 7th, 14th, 30th day after MCAO. The rFA values in the ipsilateral cortex and striatum of MCAO rats were significantly decreased compared with the sham group ( P<0.05 or P<0.01). The rFA values in cortex and striatum in focal cerebral ischemia rats of BYHWD group were higher than those in the model group, while only 14 d showed significance ( P<0.05 or P<0.01). Conclusions:T2WI and DTI clearly provided the informations of the location and morphology of the infarct areas. The ischemic brain showed significant infarction, edema, and white matter injury, which were ameliorated with time going on. BYHWD reduced the infarction percentage, inhibited cerebral edema and stimulated the recovery of neurofibra, suggesting that BYHWD could protect against cerebral ischemia.

ABSTRACT

Objective:To evaluate the clinical efficacy of Buyang-Huanwu Decoction combined with modified Tingli-Dazao-Xiefei Decoction on the basis of conventional western medicine therapy in the treatment of chronic heart failure (CHF) with syndrome of qi deficiency and blood stasis. Methods:Seventy patients who met the inclusion criteria from November 2017 to November 2019 in Shijingshan District Hospital of Traditional Chinese Medicine were randomly divided into two groups, 35 in each group. The control group was treated with western medicine for chronic heart failure, and the treatment group was treated with Buyang-Huanwu Decoction combined with Tingli-Dazao-Xiefei Decoction on the basis of the control group. Both groups were treated for 2 weeks. The TCM syndrome scores were observed and compared before and after treatment. Minnesota Living with Heart Failure Questionnaire (MLHFQ) was used to evaluate the quality of life of patients. Lee’s Heart Failure Score was used to evaluate the severity of symptoms. The N-terminal pro-B type natriuretic peptide(NT-proBNP) was detected by ELISA. The adverse reactions during treatment were observed and the clinical efficacy was evaluated. Results:The total effective rate was 91.4% (32/35) in the treatment group and 77.1 % (27/35) in the control group, and the difference was statistically significant ( χ2=7.050, P=0.014). After treatment, the serum NT-proBNP in the treatment group [(1 725.3 ± 1 473.8) ng/L vs. (2 485.7 ± 2 164.4) ng/L; U=341.200, P=0.031] was significantly lower than that of the control group. The NT-proBNP [(54.3 ± 26.7) % vs. (35.5 ± 19.8)%; U=4.310, P=0.003] was significantly higher than that of the control group. After treatment, the TCM syndrome scores and MLHFQ scores in the treatment group were significantly lower than those in the control group ( t=3.785, 9.925, P=0.031, 0.001). During the treatment, no obvious adverse reactions were observed in both groups. Conclusion:On the basis of standardized treatment of Western medicine, Buyang-Huanwu Decoction and Tingli-Dazao-Xiefei Decoction can improve the clinical efficacy of CHF patients with qi deficiency and blood stasis syndrome, promote the repair of damaged myocardium (reduce NT-proBNP), and improve the quality of life of patients.

ABSTRACT

Objective To develop the UHPLC-MS/MS method for the determination of amygdalin, paeoniflorin, ferulic acid, calycosin glucosidase, quercetin and formononetin in Buyang-Huanwu decoction. Methods Isocratic elution was carried out with mobile phase consisting of methanol- 2 mM ammonium formate. The separation was performed on Agilent ZORBAX SB-C18 maintained at 35 ℃. The flow rate was 200 μl/min, and the injection volume was 2 μl. The mass spectrometer was operated in the positive and negative ionization electrospray (ESI) mode using multiple monitoring (MRM) for analysis of six components. The mass spectrometric conditions were that ion source temperature 400 ℃, dry gas flow 500 L/h, atomization gas flow rate 75.8 Kpa, spray voltage 4000 V, dry gas temperature 400 ℃. Results The amygdalin, paeoniflorin, ferulic acid, calycosin glucosidase, quercetin and formononetin were all analyzed exactly, and the linear ranges were 0.5-32, 0.2-12.8, 0.1-6.4, 0.8-51.2, 0.4-25.6, 0.08-5.12 ng, respectively. The r were 0.9921, 0.9945, 0.9928, 0.9958, 0.9947, 0.9966, respectively. The recoveries of six analytes ranged from 99.21% to 101.44% and the relative standard deviations were all below 2.05%. Conclusions A sensitive, accuracy and suitable UHPLC-MS/MS method has been developed, and the method could be applied for the determination of amygdalin, paeoniflorin, ferulic acid, calycosin glucosidase, quercetin and formononetin in Buyang-Huanwu decoction.

ABSTRACT

Objective To observe the effects of Buyang-Huanwu decoction (BYHWD) combined with enriched environment (EE) on the abnormal activation of astrocytes and expression of angiogenic factors in rats with cerebral ischemia. Methods The rat model of permanent middle cerebral artery occlusion (pMCAO) was established by using suture method. Male SD rats were randomly divided into sham, model, EE, BYHWD and BYHWD+EE group. Rats in the BYHWD and BYHWD+EE groups were intragastrically adminstratered with BYHWD (16.1g/kg) 24h after MCAO and once daily for consecutive 15 days. Accordingly, rats in the sham, model, and EE groups were adminstratered with the sam volume normal saline. Rats treated with EE and BYHWD+EE were housed in the enriched environment 12 h every day. The EE, BYHWD and BYHWD+EE group rats were given relative treatment continuously for 15 days. Beam walking test was performed to examine the motor function of rats. Next, the HE staining was conducted to detect the pathological alterations of the brain. Immunohistochemical staining with DARPP-32 and GFAP were applied to respectively observe the changes of multiple spinous neurons and activation of astrocytes in the striatum. Subsequently, the mRNA expressions of angiogenic factors including VEGF/VEGFR2 and Ang1/Ang2 were detected by RT-PCR. Results The rats treated with BYHWD+EE revealed markedly elevated beam walking scores on the 7th and 15th day after MCAO compared with model group (P<0.01). Compared with the model, BYHWD+EE treatment significantly increased the number of neurons (31.08 ± 8.06 vs. 19.00 ± 9.12, P<0.01), elevated the number of DARPP-32-positive cells (975.3 ± 589.68 vs. 271.25 ± 164.98) and integral optical density (18621.87 ± 6996.64 vs. 4071.60 ± 2477.27) of DARPP-32 (P<0.01), suppressed the expression of GFAP (6094.07 ± 3211.74 vs. 10002.91 ± 8430.34, P<0.01) and up-regulated the mRNA levels of VEGF (1.59 ± 0.80 vs. 0.77 ± 0.33), VEGFR2 (1.58 ± 0.88 vs. 0.70 ± 0.37), Ang1 (1.58 ± 0.52 vs. 0.84 ± 0.13), and Ang2 (1.25 ± 0.25 vs. 0.90 ± 0.22) in the striatum (P<0.05 or P<0.01). Conclusions The Buyang-Huanwu decoction combined with enriched environment exerted synergistic effect on ischemic rats and promoted motor function by inhibiting the excessive activation of astrocytes and improving the expression of angiogenic factors.

ABSTRACT

Objective To study the effect of Buyang-Huanwu decoction on incidence of macular edema, visual acuity and serum VEGF, IL-6 levels after operation in diabetic cataract patients. Methods A total of 46 post-operative patients with diabetic cataract were randomly divided into two groups. The control group was treated with pranoprofen eye drops combined with tobramycin dexamethasone eye drops, and the study group was treated with Buyang-Huanwu decoction combined with Salviae Miltiorrhizae and Ligustrazine Hydrochloride injection based on the control group. The central subfield mean thickness (CSMT) at baseline before operation and 1d, 1w, 4w, 8w, 12w after operation, corrected vision at 0w, 1w, 6w, 12w, serum levels of vascular endothelial growth factor (VEGF) and interleukin-6 (IL-6) before and after treatment of two groups were observed, and the incidence of macular edema was analysed. Results The CSMT of study group at 8 w (226.35 ± 9.88 μm vs. 214.74 ± 9.27 μm, t=2.118) and 12 w (233.57 ± 11.25 μm vs. 213.62 ± 10.13 μm, t=2.258) after operation were significantly lower than those of control group (P<0.05). The best corrected visual acuity of study group at 6 w (4.61 ± 0.32 vs. 4.82 ± 0.25, t=2.0460 and 12 w (4.57 ± 0.29 vs. 4.79 ± 0.24, t=2.180) after operation were significantly higher than those of control group (P<0.05). The levels of VEGF (174.31 ± 45.63 pg/ml vs. 83.65 ± 24.47 pg/ml, t=4.145) and IL-6 (27.25 ± 9.21 pg/ml vs. 15.24 ± 3.14 pg/ml, t=3.829) of study group were significantly lower than those of control group (P<0.05). The incidence of macular edema at 8 w and 12 w after operation in study group were respectively 0.0% and 2.9% (1/35), and the control group were 16.2% (6/37) and 21.6% (8/37). The difference was statistically significant (P<0.05). Conclusions The Buyang-Huanwu decoction in treatement of post-operation patients with diabetic cataract could reduce the incidence of macular edema and improve the visual acuity.

ABSTRACT

Objective To investigate the effect of Buyang-Huanwu decoction on left ventricular diastolic function in DHF rats.Methods Thirty male Sprague-Dawley rats were randomly divided into the normal group (n=10),the Chinese medicine group (n=6),the western medicine group (n=6) and the model group (n=6).Except for the normal group,the other groups got abdominal aorta stenosis.After 12 weeks,The Chinese medicine group was given Buyang-Huanwu decoction 12.72 g/(kg·d),and the western medicine group was given metoprolo 1 0.004 5 g/(kg·d),and the normal group and the model control group were given water.After once daily for 8 weeks,echocardiography was used to measure EF,E/A ratio and e/a ratio.The heart rate,left ventricular diastolic pressure,left ventricular end-diastolic pressure,tension and maximal change in pressure of left ventricular were measured by Langendorff.Results Compared with the model group,the e/a value (1.11 ± 0.28 vs.0.67 ± 0.20) of the Chinese medicine group significantly increased (P<0.05),and the ventricular muscle tension (0.86 ± 0.64 g vs.1.01 ± 0.33 g) significantly decreased (P<0.05).Conclusions Buyang-Huanwu decoction could improve cardiac relaxation by reducing ventricular muscle tension,thus improving diastolic function of diastolic heart failure rats.

ABSTRACT

Objective To investigate the effects of Bu Yang Huan Wu Tang on serum contents of microelements sunch as Zn,Cu,Fe,Se and Mn in spontaneously hypertensive rats-stroke prone(SHR/SP). Methods Twenty male SHR/SP of 8 weeks old were divided into two groups: treatment group(n=10),treatmemt with Bu Yang Huan Wu Tang;control group,taking normal feed.Ten WKY rats were served as blank group. The experiment lasted for three months,and the monitored parameters were serum contents of microelements such as Zn,Cu,Fe,Se and Mn. Results It was revealed that the serum contents of Zn,Mn and Se of the blank group were significantly higher than those of the treatment group(P0.05). Conclusion Bu Yang Huan Wu Tang may have regulatory effects on the serum contents of microelements such as Zn,Cu,Fe,Se and Mn in SHR/SP.
